Use your corporate email address. Requests are reviewed by a 6101 FIX administrator — you will receive a response by email once a decision has been made. This form is for rail operator staff only, not for passengers.

Must be your corporate email — personal addresses will not be accepted.
This will be visible to passengers when you post updates on incidents. Use something you are comfortable with — your first name and initial, or your role and first name, works well.
